Also, I understand there was originally a feather item you could get, which allowed you to jump (like in Mario Kart (SNES), but was removed because it allowed for too many glitches and unfair short cuts.

Going only by photos in Nintendo Power:

Kamek was in one proto, whose screenshot was shown in Nintendo Power.

Toad was named "Kinopio" in a later proto, one much closer to the (J) release version, whose screenshot was shown in Nintendo Power. (Kamek had already been replaced with Wario by this time.) Without this screenshot, I would have never rediscovered Pinocchio.

From what I understand, both are true.  MagiKoopa (a.k.a. Kamek, or the wizard koopa) was a playable character, though it wasn't clear if he replaced Donkey Kong or if Donkey Kong was in the proto and there are just no pictures of him.

As for the feather, it makes sense to remove it.  Really it was almost a "patch" item to do something that you couldn't normally do in a Mode 7 SNES game -- jump your character. Once the game was translated into a truly 3D format, jumping became a non-issue and, as was pointed out, actually created more problems than it solved in terms of glitches.

'Course the jump sometimes created glitches in the SNES game too -- the error correction was just better.  Remember when Lakitu had to "fish" you out of areas you couldn't be in?  That was game error correction.

Here you go.   Pre-release footage:

Short clip, so don't hesitate to use the pause button.

Oh, and since NOA put it on the N64 promo tape (that's what the clip is from), odds are there is a complete pre-release review cart floating around out there somewhere.

Keep your eyes open.   :o
