I keep all my 5 screw games (all my games actually) in an excel spread sheet with info about the variations.
Anyways, here is what I have for 5 screw games if its of any help information wise.

Title = title of the game
Company = company that produced/distributed it
Code = the NES code (all my 5 screw games by Nintendo have no code)
Made In Japan = whether or not the label has made in japan somewhere on it. Mighty Bomb Jack for example has a version with and a version without.
Cart Type = these are all 5 screw carts
Label variation = just a description. For example some of my Slalom carts have a copyright synmbol, and others have Copyright spelled out. If I put something in brackets, it means its a symbol.
